Pre-planned Absence Form  If a student must miss school because of a medical or dental appointment, or other special circumstances, please fill out this form and turn it in before the date(s) of absence. 


Asthma Medication Authorization Form
If a student uses an inhaler to take prescription medication while under the supervision of the school, this form must be filled out, per Wisconsin state statute.

Excuse Form
If a student missed school due to illness, bad weather or other circumstances, please fill out this form after the absence has occurred. (The download allows for reporting of two absences per page; please cut to size.)  


Lakeside Handbook This document, contained inside the 2012-13 student planner, explains attendance and behavioral requirements for Lakeside students, among other info. Use of electronics, dress code, health and medication guidelines are also included.
Changes to the Lakeside Handbook for 2013-14 This pdf document highlights the changes that go into effect Fall 2013.


Senior picture submission guidelines
: the yearbook needs senior photos that follow certain size, orientation, pose and background rules. Please take a look at the form for the guidelines and take the sheet along to any senior photo shoot you attend.

Visitor Form If a student would like to bring someone who wants to visit the school and may eventually become a student of LLHS, fill out this form and turn it in prior to the desired visit date.

Registration Night Forms

Families can access several forms needed for registration at Lakeside. Forms needing families to fill in information can be filled out electronically, either in fillable pdf documents or a Word document (free & reduced lunch). Print out completed forms, add handwritten signatures as needed and bring them along on registration night! (And for those who are thoughtfully wondering, yes, it's OK that they'll all be on white paper!)

Junior Parents:
A letter and reply form requesting your help to make May Banquet a wonderful memory for the seniors was given to all junior students. Please download, read and return this letter and form with your ability to serve if your junior did not bring it home.

What is the May Banquet schedule of events?

What are the dress code guidelines?

Formal event (such as May Banquet) attire should not show cleavage or abdomens. See-through dresses are not allowed. No cut-outs are allowed on the sides or front panels of dresses. Backless dresses should be waist-line height or higher with appropriate coverage on the sides. These dress code guidelines also pertain to non-LLHS students who may be attending formal dances as guests. Failure to comply with these guidelines will result in the student/guest not being admitted to the dance.

For the dance, are there any rules or guidelines? Yes.
Please carefully observe the following guidelines so that you will not be removed from the dance and embarrassed…

  • While dancing, it is appropriate to keep your hands above the waist, but not to touch an individual in a sexual way (or to give the impression that you’re doing that).
  • “Dirty dancing,” as it is sometimes called, front-to-back dancing, or “pelvic grinding” is not appropriate.
  • Running into people on the dance floor, or putting people on your shoulders, is not acceptable, or in most cases even safe.
  • Chaperones will intervene in any inappropriate situation. The directions they give are authoritative. Chaperones may expel a student from a dance who does not show Christian decency or violates the Student Handbook. Parents will be contacted if their child is expelled from a dance.
  • If a person or couple needs to be warned more than once, they may be asked to leave the dance or sit out for the remainder of the dance.
  • Continued defiance of school rules may result in a student, or couple, being banned from future dances.

There are copies of these guidelines in the office. If a student is bringing someone from off-campus, it’s probably a good idea to pick up a copy of these guidelines in the office closer to the date.

Semester Exam Excuse Form

Download the most recent semester exam excuse form here. Students are required to be at Lakeside during semester exams. However, there are certain classes (a phy ed class, for example) in which an exam or closing activity will not take place; instead, it will be a study hall. 

If that class takes place at the beginning or end of the exam day, that student may be excused. Students who want to be excused from such classes must fill out a "semester exam excuse form" and have it signed by parents in order to be excused. Students who do not do this and are missing from their classes will be considered "unexcused." 

Extra forms can be found in the office. If you have any questions, please don't hesitate to call the school office. A student who plans to be present at Lakeside during all exam periods does not need to hand in a form.
